Your First Visit
Your first visit is an important step to finding out the best treatment plan for you! Please fill out our intake form and the worksheet prior to your first visit. You may either use the interactive forms on our website or request to have them emailed to you prior to your first appointment. These forms supply the information that we need to create your patient account in our system and provide valuable information regarding the symptoms you have experienced and the treatment options you have tried. This will assist the Physician in understanding your history and performing your examination.
You will also receive our policy disclosure statements, HIPPA acknowledgement, and any appropriate consent forms that are needed for your first visit. We ask that you provide a photo ID, your insurance card, and any special offer documentation that you have so that we may place a copy in your chart.
If you are scheduled for a free screening, you will receive a basic examination designed to determine if you have venous disease. We will perform a limited ultrasound examination to determine if you are a good candidate for laser therapy. The findings of the ultrasound examination will be discussed with you and we will explain the basics of Endovenous Laser Treatment (EVLT) and answer any questions you may have. We will give you enough information about your disease and treatment options available to be able to make an informed decision as to whether or not you wish to pursue treatment. If we find that you are not a good candidate for EVLT or if you decide you are not interested in pursuing treatment at this time, your free screening will be completed and there will be no charge to you.
If, after receiving sufficient information, you decide to proceed with treatment, we will attempt to convert the free screening to a consultation and mapping visit if time permits. We may be required to set up a consultation appointment on a different day if the schedule does not allow the time to perform a more detailed examination and teaching. Please note that a consultation and mapping visit is not part of the free screen and will generate a bill which will be presented to your insurance company. Many patients start with a consultation and mapping visit rather than a free screen if they are interested in receiving treatment for their venous disease.
Your first spider vein treatment appointment is important as a brief screening will be performed to determine if you are likely to have underlying venous disease contributing to the development of your spider veins. Our nurse will assess your history, symptoms, and the location of your spider veins. If signs of possible venous disease are present, she will recommend that you see a vein specialist prior to having your spider veins treated.
